Map a normal day
Walk through preparation, arrivals, clinical handoffs, checkout and follow-up with representatives from each team. Record what information changes at every step and where work pauses. Start with an ordinary visit, then identify the interruptions that regularly create duplicate work.
Define completion consistently
A scheduled visit, an attended visit, signed documentation and settled billing are different outcomes. Give each task an explicit completion rule and owner. Avoid a single status that different teams interpret differently or that allows unresolved work to disappear from view.
Use exception queues
Separate missing information, unanswered enquiries, recall follow-up and financial issues. Give each queue an owner, a review rhythm and a way to record the next action. A queue should support a decision rather than become a permanent list with no responsibility attached.
Review and refine
Choose a small number of measures and reconcile them with actual work. Ask staff which steps remain confusing and whether the software is creating unnecessary duplication. Change the process deliberately, document what changed and compare outcomes using consistent definitions.
