Define the cohort
Choose a period and document what counts as a no-show, late cancellation and rescheduled visit. Segment by appointment type, lead time and confirmation status where those data are reliable. Review records before drawing conclusions from a dashboard total.
Find communication failures
Check whether the clinic had the correct contact details, whether reminders were delivered and whether patients could reply or cancel conveniently. Assign failed messages to a staff review process. A sent notification alone does not show that a patient received or understood it.
Use proportionate changes
Discuss clear appointment expectations, convenient confirmation and a consent-based short-notice list with the team. Any deposits or cancellation policies need appropriate review and clear communication. Consider access barriers and individual circumstances instead of applying the same intervention to every patient.
Measure the result
