At a glance
Digital dental forms collect information and preserve the version that was submitted. A complete workflow includes identity, accessibility, staff review, signature context and retention; a simple online questionnaire does not establish all of these controls.
The private prototype supports saved patient profiles, scheduling, visit statuses and recall tracking. Clinical, billing, insurance, messaging and website integrations described in our roadmap are not yet available for clinical use.

Use a fictional submission with a long answer, a skipped required field and an interrupted connection. Check labels, keyboard access and error messages. Decide whether a patient can save progress and how a team handles someone who needs a paper or assisted process.
Retain the submitted version
Change the form wording after a submission, then retrieve the earlier record. Ask whether the stored copy includes the wording, responses, signer and time applicable to that submission. Corrections should remain distinguishable from the original information.
Assign the review step
A completed form may still need staff or clinician review. Ask where changed information appears, who is responsible for checking it and how an urgent concern is escalated through the clinic’s established process. Submission alone should not imply clinical review.
Current availability
The prototype records an administrative intake-progress label. It does not yet collect medical histories, signatures or patient consent forms. The public enquiry form is for business questions and must not be used for patient information.

Does an intake status prove a signed form exists?
No. Verify the underlying document, submitted version and review history separately from a dashboard label.
